Acceptable Third-Party Documentation Requirements to Request an Exception to the Withdrawal Policy Due to Extenuating and Unforeseen Circumstances

  • All third-party documentation specify when it is written, refer to your enrollment dates and assert that the circumstances did not pre-exist the term start.
  • For medical issues or serious injury directly involving yourself or immediate family member, you must submit a signed doctor’s statement on his/her official letterhead. The statement must include dates seen by the doctor, medical condition, length of treatment and how illness/injury prevented you from attending classes.
  • For death of an immediate family member or someone for whom you are the primary caretaker, a copy of the obituary or death certificate is necessary.
  • For other severe and unforeseen circumstances that forced you to withdraw, a signed statement on official letterhead from a third party (e.g. employer or legal entity). The statement must include dates, circumstances and how the circumstances prevented you from attending classes.
