Identity and Statement of Educational Purpose
Students may be selected to verify their identity and sign a statement of educational purpose. If chosen to complete this process, financial aid will not be determined at Empire State College until this requirement has been satisfied. If requested, please download the appropriate form below.
UPDATE: Due to COVID-19 and the continued closure and/or limited access to college locations, students may submit the completed signed form below instead of visiting a college location in person to complete the requirement. At this time it does not need to be notarized. Students must include a copy of their identity documentation.
